"Informed Consent" statements?

Gallup wisely requires that participating organization assume responsibility for "informing individuals that the institution will have access to their results" as well as how those results will be used. I would be interested in seeing some "informed consent" statements from institutions that have been at this for awhile.

Hi Chris - have you found any models yet?
Maybe now that the school year has begun, folks will be more tuned in to developing/sharing their informed consent documents.

Gallup has beaten me to the punch by providing an informed consent statement at the login prompt which covers most contingencies. I'd be curious to know if schools are still planning to use their own statements.
